Southwest Midwives provides childbirth care and women's health care.
Our childbirth care includes:
- Prenatal care
- Nutrition and exercise counseling
- Labor support and birth of baby at Mercy Medical Center
- Water therapy in labor, including waterbirth
- Postpartum care and breastfeeding support
- All pain relief options available
Our women's health care includes:
- Annual exam
- Pap smear
- Birth control
- Breast examination
- Preconceptional care
- Pregnancy testing
- PMS counseling
- Diagnosis and treatment of vaginal and urinary tract infections
- Resource and referral service
- Hormone replacement therapy
- Menopause counseling
- Treatment of minor illnesses
- Sports physicals
Please schedule your appointment as far in advance as possible. When you call, please give the receptionist as much information as possible about your visit so she can schedule enough time during the visit to meet your needs. This information is kept confidential within our office.
If you need to cancel a visit, please provide as much notice as possible so we may schedule other clients as needed.
Let us know of changes in your address or phone number so that we can reach you as needed. Also, please notify us of insurance changes so that billing will be done correctly.
We respect your desire for confidentiality. We can't release any information regarding your care without your written consent. Only records generated in our office can be released. Please give us five working days to process your medical record requests.
Insurance, Fees and Payments
We accept many insurance plans, including Colorado, New Mexico and ER Medicaid. Financing plans are available for self-pay.
If you have questions about whether we are contracted with a particular insurance company, feel free to call our office any time to check and verify your insurance with our practice. The most efficient way to verify your insurance is by giving your insurance company our tax ID number. You may call our office to get this number. Our billing department number is 970-259-7679.
We can bill insurance companies as a courtesy to you. If you are a member of one of our contracted insurance companies, we will collect any applicable co-payments at the time of service. If your insurance company is not one with whom we have contracted, we will collect that portion of the bill that is your responsibility at the time of service. You may pay by cash, check, MasterCard or VISA.
Special circumstances may require a payment plan; please make these arrangements in advance by contacting our front office during regular office hours.
Please know the terms of your insurance. This is very important information so we will know what to expect from you and/or the company in terms of payment. Be sure to bring your insurance card to your appointment.
